Please take the time to nominate a friend, colleague, client, employee, business, event, or employer who deserves to be celebrated! Consider nominating an individual or organization whose accomplishments have yet to be publicly acknowledged.
How do I submit my nomination?
Use the online form
This year, you can submit your nomination through an online form. Each award has its own nomination form.
All you have to do is click on the title of the award relevant to your nomination, and you will be linked to a description of the award, the nomination criteria (who is eligible) and the factors that will be considered in the evaluation process.
You will also find both a PDF version and a Rich Text File version of the award’s nomination form which you can print for reference before you begin to fill out the online form.
You fill out the information fields provided in the form and can also attach documents to the form, including photos. Then you simply click Submit Your Nomination.
After you submit your nomination, you will be able to print your completed form. Click on the bar which says Click Here for Printable Page and then click on Print This Form.
IMPORTANT NOTE: You will need to complete the nomination form all at once and then submit it.
There is no provision for filling out parts of the form and saving the document to be completed at a later time. We encourage you to have all your information ready to insert in the form before you begin to fill it out. You can paste text into the boxes.
Other Ways to Submit Your Nomination
We encourage you to use the online nomination forms. However, if you would like to create your own document in a Word or PDF file you may do so. Please remember to include all the information fields that are in the nomination form contained on the website to ensure your nomination qualifies.
