Real Plans for Real Life program
Citizen Advocacy's Real Plans for Real Life program supports adults with developmental disabilities in choosing and designing a life for themselves. We believe that we all have the right to be in charge of our own lives. Using Person-Centered Planning tools, a life plan will be developed based on an individual's goals, dreams, strengths, and abilities. Translating the plan into real life includes creative problem solving and considering new ways of accessing resources and supports.
Here's how it works:
- Service Coordination identifies individuals considered most in need and refers them to Citizen Advocacy's Real Plans for Real Life program.
- Developing a Person-Centered Plan begins by meeting with the individual and the significant people in his or her life.
- Together, we gather information using creative tools designed to assist the individual to think about his or her dreams in the long term.
- This information helps us discuss the individual's support needs, goals, and life objectives.
- Our conversations lead to the development of a realistic and attainable short-term and long-term Person-Centered Plan.
- After a Person-Centered Plan is developed, we support the individual in making the plan a reality.
- We help to assess the community supports and services necessary to fulfill the individual's goals and dreams.
- Working collaboratively with a wide variety of supports, we offer education and information as needed for effective implementation of the plan.
- A large part of our role is to empower the individual and his or her support circle to manage and coordinate the individual's life.
- Even though there may be bumps along the way, we assist the individual, monitor his or her plan, and ensure that adjustments to the plan are made as necessary.
- Creating and implementing a plan takes time; living a real and complete life is its own reward.
